INTRIGUING RESULTS REGARDING THE PREVALENCE AND DISTRIBUTION OF HPV TYPES IN WOMEN WITH CERVICAL CANCER IN A ROMANIAN POPULATION

Rezumat: Rezultate intrigante privind prevalenţa si distribuţia tipurilor de HPV în rândul femeilor cu cancer de col uterin din România

 

Scopul: Este bine stabilită relaţia de cauzalitate între infecţia persistentă cu HPV şi cancerul de col uterin. Prevalenţa infecţiei cu HPV în rândul femeilor din România nu a fost atent studiată.

Materiale şi Metode: 80 de cazuri de cancer invaziv de col uterin confirmat histologic, neiradiate anterior, au fost selectate din rândul femeilor din România, efectuându-se genotiparea HPV la acestea. Detectarea s-a bazat pe izolarea de fragmente de ADN, amplificarea PCR a ADN-ului ţintă, hibridizarea produselor PCR cu oligonucleotide şi identificarea colorimetrică a produselor. O analiză statistică a fost efectuată.

Rezultate: Prevalenţa infecţiei cu HPV a fost de 87,5%. Dintre cazurile de cancer de col uterin cu HPV-ADN pozitiv, majoritatea cazurilor au prezentat infecţia cu un singur tip de HPV(87,14%); la 7 paciente s-au detectat 2 tipuri de HPV(10%); şi la 2 paciente au fost descoperite 3 tipuri diferite de HPV. HPV16 a fost cel mai des întalnit tip, cu 72,5% contribuţie relativă ca unică infecţie(58 paciente) sau multiplă. Nu a fost indentificată o asociere semnificativă din punct de vedere statistic între genotipul HPV, vârsta pacientelor, tipul histologic sau stadiul bolii(p>0,05).

Discuţii: Rezultatele noastre sunt intrigante, cu o prevalenţă mare a tulpinii HPV16(72,5%) şi 10 alte cazuri de tipuri de HPV cu risc crescut, cu o prevalenă scăzută(1,3-2,7%) pentru fiecare. Infecţia cu multiple tipuri a fost detectată la 12,85% din femei. HPV18 a fost detectat la o singură pacientă, în ciuda prezenţei a 4 adenocarcinoame şi a unui carcinom neuroendocrin..

Abstract

Purpose:. Persistent HPV infection is a well-established cause of cervical cancer. The prevalence of HPV infection among Romanian women has not been broadly studied.

Material and Methods. Eighty histologically confirmed invasive cervical cancer specimens of previously non-irradiated Romanian women were collected and a HPV genotyping was performed. Detection was based on DNA isolation, PCR amplification of target DNA, hybridization of PCR products with oligonucleotide probes and colorimetric identification of products. A statistical analysis was performed.

Results. HPV-DNA prevalence was 87.5%. Among HPV-DNA positive cervical cancers, most of the cases presented as single infections (87.14%); 7 patients harbored 2 HPV types (10%); and in 2 patients, 3 different HPV types were discovered (2.85%). HPV16 was the most detected type, with 72.5% relative contribution as single (58 patients) or multiple infection. It was not found any statistical association between the HPV genotype, patients’age, histology of the tumour or stage of disease (p>0.05).

Discussion: Our results are intriguing, with a high prevalence of HPV16 (72,5%) and 10 other high risk HPV type, with a low prevalence (1.3-2.7%) for each one. Multiple infection was detected in 12.85% of women. HPV18 was detected in only one patient, despite the presence of 4 adenocarcinoma and one neuroendocrine carcinoma histologies.
