Rezumat
Obiective: Scopul acestui studiu a fost evaluarea eficacităţii şi complicaţiilor tehnicii chirurgicale TVT în tratamentul incontinenţei urinare de efort la femeie.
Metodologie: Acest studiu a fost unul unicentric retrospectiv şi a inclus toate pacientele care au fost operate pentru incontinenţa urinară de efort pe o perioadă de 10 ani în serviciul de obstetrică-ginecologie din Mulhouse (Franţa), prin tehnica TVT. Incontinenţa urinară a fost clasificată în 4 stadii, iar preoperator toate pacientele au beneficiat de un bilanţ urologic. Durata medie de urmărire a pacientelor a fost de aproximativ 12 luni postoperator.
Rezultate: Au fost incluse în studiu 76 paciente, cu o vârsta medie de 58 de ani şi limite între 34 şi 84 de ani. Majoritatea pacientelor se aflau în stadiile 2 şi 3 de incontinenţă urinară (63,2%); incontinenţa urinară de efort a fost de 69,7%, mixtă 28,9% şi un singur caz de incontinenţă prin imperiozitate. Dintre paciente 74% au fost considerate vindecate şi 13,7% ameliorate în postoperator. La un număr de 23 de paciente (30,26%) au apărut complicaţii postoperatorii tardive, majoritatea fiind reprezentate de infecţiile urinare şi retenţia de urină.
Concluzii: Tehnica chirurgicală TVT este o metodă eficientă în tratamentul incontinenţei urinare de efort la femeie, cu rezultate bune şi foarte bune în peste 85% din cazuri. Prezenţa complicaţiilor nu modifică rezultatele la distanţă în ceea ce priveşte continenţa urinară. Vârsta nu pare să influenţeze rezultatele, cu excepţia pacientelor peste 70 de ani, unde eficienţa pare să diminueze. Prezenţa unei insuficienţe a sfincterului uretral a modificat uşor eficienţa rezultatelor, acestea rămânând însă satisfacatoare. Stadiile avansate de incontinenţă urinară au diminuat semnificativ rezultatele postoperatorii.
Abstract – The tension-free vaginal tape technique for the treatment of stress urinary incontinence in women – results of a study of 76 patients
Aims: The aim of this study was to evaluate the effectiveness and the complications of the TVT surgical technique in the treatment of stress urinary incontinence in women.
Methods: This study was a single-center retrospective study, including all patients operated for stress urinary incontinence during a 10 year period in the service of obstetrics-gynecology in Mulhouse (France), using the TVT technique. Urinary incontinence was classified into 4 stages, and all patients underwent a urological evaluation preoperatively. The mean follow-up of patients was approximately 12 months postoperatively.
Results: The study included 76 patients with a mean age of 58 years, age limits between 34 and 84 years. The majority of the patients were at stages 2 and 3 of urinary incontinence (63.2%); 69.7% of the patients had stress urinary incontinence, 28.9% mixed urinary incontinence, and there was one case of urge urinary incontinence. Of all patients, 74% were considered healed and 13.7% improved during the postoperative period. 23 patients (30.26%) had late postoperative complications, of which the majority consisted of urinary infections and retention of urine.
Conclusions: The TVT surgical technique is an effective method for the treatment of stress urinary incontinence in women, with good and very good results in more than 85% of the cases. The presence of complications did not change late results regarding urinary continence. Age did not seem to influence the results, except for patients aged over 70, in whom effectiveness seemed to be lower. The presence of a failure of the urethral sphincter slightly altered the effectiveness of the results, which remained satisfactory. The advanced stages of urinary incontinence significantly diminished postoperative results
